Review process and documents for practitioners

All Education, Health and Care (EHC) plans must be reviewed at least annually. By law, it is the Local Authority's (LA) responsibility to complete the review process, but we delegate some steps to the school/setting/or other education provider, who are known as the host.

Annual Review documents

The Annual Review (AR) is more than just a meeting, it is a complete process, including actions that must be taken before and after the meeting. This process must be completed on or before the anniversary of when the EHC Plan was first issued or the anniversary of the last review. For looked after children, best practice is that the annual review should, if possible and appropriate, coincide with the Personal Education Plan (PEP). This is laid down in law and in the .

You should use the ÌÇÐÄvlog¹ÙÍø EHC plan statutory review form (DOC) for all reviews regardless of the type of review, the child/ young person's age, key stage, or placement.

ÌÇÐÄvlog¹ÙÍø have recently (January 2025) introduced a Health Questionnaire (DOC) to help obtain a complete picture of a child/ young person's wellbeing and mental health. Please circulate this prior to the Annual Review meeting.
